If you are considering residential care for yourself or someone else, the following information is provided as a guide to the ‘First steps’ in the process. To be eligible for residential care, it is necessary that you have an Aged Care Assessment; this can be achieved by contacting an Aged Care Assessment Team. There are ACATs in all states and regional areas throughout Australia. Tasmanian ACATs can be contacted as follows:
- Northern Team: 6336 4144
- Southern Team: 6222 7274
Without an ACAT assessment, only limited further assistance can be provided as the assessment provides a report on your care needs and the level of care you require or are eligible for. On completion of the ACAT assessment, you will receive an information pack called ‘5 Steps to Entry into Residential Aged Care’ – this pack has 2 important documents to complete:
- Application for Respite Care or Permanent Entry to an Aged Care Home
- Request for an Assets Assessment – This assessment will determine the level of fees and charges you will be required to pay.
Further information about the assets assessment, together with the Request for Assets Assessment Form and the Application for Respite or Permanent Care can be downloaded and printed from the links below.
It is strongly recommended that you consult your financial planner or accountant in regards to the financial implications of the assets assessment. To ‘put your name down’ with us for residential care, we conduct pre-admission appointments where you will be taken through waiting lists, costs and other procedures to enable a decision about future admission into one of our facilities. Most importantly though, you will be able to ask those questions that you are concerned about and learn more about life in Residential Care, plus tour our facilities. At this appointment you will also meet members of the nursing and care team who will talk about your care needs. As a preliminary step, here’s our current information brochure and pricing table about our residential facilities in Hobart, Legana and Norwood:
