Presbyterian Care Tasmania provides transition care places under a formal arrangement with the Tasmanian Health Service in accordance with the provisions of the Commonwealth “Transition Care Program Guidelines (2015)”.
Transition Care is a time limited program up to a maximum of 12 weeks and to be eligible you must be a current hospital in-patient and have been assessed by the Aged Care Assessment Team’s (ACAT) approved assessment process as requiring transition care on discharge following an acute and/or sub-acute episode.
One of the requirements of the program is that Presbyterian Care Tasmania is required to offer you a formal agreement covering the details of your stay with us including the fees payable. Details of any fees payable and a formal agreement will be offered before admission and the document below is in a form that meets the requirements of the Commonwealth legislation relating to the provision of transition care.

COMPLIMENTS AND COMPLAINTS
As a client of the Transition Care Program, we would like to hear from you regarding any compliments, service improvements and complaints. You are entitled to make a formal complaint about our services and we have a process in place to assist you and have any matters dealt with in a timely and confidential manner. You are also able to have your complaint dealt with by the Aged Care Complaints Scheme (Commonwealth) or referred to the Health Complaints Commissioner (Tasmania).
